What is AERS?
AERS provides assistance to functionally disabled individuals who are at risk of being placed in an institution. AERS staff conducts a comprehensive evaluation to identify services available to help the individual to remain in the community, or in the least restrictive environment, while functioning at the highest possible level of independence and personal well-being.
How Does AERS Help?
The AERS teams consist of a nurse, social worker, and as needed psychiatrist and psychologist. These teams provide the individual with a comprehensive evaluation, including medical/nursing, environmental and psychosocial assessments. The evaluation is conducted in the home setting or the individual’s current location. Upon completion of the evaluation, the multidisciplinary team develops a plan of care which recommends services needed to help the individual remain at home, or in the least restrictive environment.
Referrals:
Referrals are accepted from any concerned individual by phone, fax, mail or in person. Referrals for Preadmission Screening and Resident Reviews generally come from hospitals, nursing facilities, private physicians or other health care providers.
Cost:
- There is no charge for evaluations.
Eligibility Criteria:
Must meet both Medical and Financial Eligibility Criteria as follows:
- Medical: at risk of nursing home placement, or institutional care
- Financial: Medicaid recipient or be eligible for Medicaid within six (6) months of placement in nursing facility
