Medical Assistance Personal Care Program (MAPC)

This Program is designed to enable individuals who are physically/mentally chronically ill, disabled or the frail elderly to remain at home with a long-term plan of care.

  • Client Eligibility (also referred to as patient or recipient)
  • Each Client must:
    • Have a valid Maryland Medical Assistance card
    • Be moderately at risk of institutionalization (nursing home, etc.)
    • Need hands-on assistance with personal care and activities of daily living
    • Be under the care of a Physician

 Are you interested in becoming a MAPC Personal Care Provider?

  • Provider Requirements:
  • Be 18 years of age or older; able to speak, read,write, and follow directions
  • Have their Physician’s certification that Provider is physically and mentally able to provide care services
  • Complete the provider application and have two satisfactory references
  • Be able to pass a criminal background investigation
  • NOT be a member of client’s immediate family (parent,spouse, child, sister, brother, in-law) or legally responsible individual for a client in your care
  • Be sensitive to the needs of others and be reliable and responsible
  • Be approved by the Personal Care Program Supervisor and the Provider Enrollment Office in Baltimore

Nurse case monitoring- supervision of personal care by a registered nurse

  • Assistance with bathing
  • dressing
  • toileting
  • meal preparation
  • eating
  • transfer and ambulation
  • medications (remind only)
  • straightening area
  • laundry
  • changing bed
  • food shopping
  • escorting to medical appointments or work
