These services are available: 

Family Centered Care Coordination: a valuable service, our CYSHCN program, can help navigate you through the health care system by giving assistance in communicating with providers and agencies that help support children with special needs. A nurse can help coordinate services, address any questions, reduce barriers for care and/or help refer care givers to other community resources or specialty services. 

Children's Medical Services (CMS) can assists families, with children from birth to 21 years of age, in completing the application for medical services. If eligible, Children's Medical Service can help provide funding to un-insured and under-insured children for special medical care.   

Health Care Transition Services: a service available to youth beginning at 12 years of age. Health care transition services will help you with possible concerns and barriers that may need addressed during the transition process from pediatrics to adult health care. 

Other services offered but not limited to:

  • support and education

  • needs and readiness assessments

  • transportation assistance

  • help in locating childcare services

  • referrals to other agency programs

  • community resources and/or specialty services
