The Community Health Services division offers immigration/civil surgeon services by appointment only.  Please call (410) 543-6943 to schedule an appointment

Immigration Services include:

• Required lab testing
• Physical exam
• Completion of the United State Citizenship and Immigration Services (USCIS) I-693 form

The physical exam and I-693 form will be completed by a Civil Surgeon. 

FEES

• Fee for lab testing and physical exam = $300.00 must be paid on the day of the appointment (this service is not covered by health insurance)
• Any required vaccines are a separate fee
