FREE PUBLIC H1N1 FLU
CLINIC SCHEDULED FOR 11/4/2009
The Wicomico County Health Department
will hold a public walk-in H1N1 vaccination clinic for
high-risk priority groups ONLY, on Wednesday November 4
2009. The clinic will be held from 1:00pm until
8:00pm or until vaccine supply is exhausted. The walk-in
clinic will be held at the Wicomico County Civic Center
located at 500 Glen Avenue, Salisbury, MD. Appointments are not needed.
Vaccinations will be available on a first come, first serve
basis. The public should expect long lines.
Individuals attending this clinic
are encouraged to print, complete and bring the the
following form with them to the Civic Center in effort to
expedite the vaccination process.
H1N1 VACCINATION CONSENT & ADMINISTRATION RECORD
The Wicomico County Health Department
is strictly following sub-priority recommendations set forth
by the Centers for Disease Control regarding vaccination
prioritization. Individuals considered to be high risk
include the following: pregnant women, persons who live
with or provide care for infants under 6 months of age;
children aged 6 months-to-4 years, and children and
adolescents aged 5--18 years who have medical conditions
that put them at higher risk for influenza-related
complications. Again, vaccine will be available only to
the above sub-priority groups.
Additional clinics for individuals
both within the priority groups and those NOT within the
priority groups will continue to be scheduled as vaccine
becomes available. Clinic dates and locations will be posted
