- Prevents inappropriate admissions of the elderly, 65 years of age or older, to any state mental health facility
- Assists with placement through social, medical and/or psychiatric evaluations if necessary
- Evaluates persons at risk of any long term institutionalization for appropriate community services to prevent or delay out of home placement
- Appointment required for evaluation; home visits made
- Hours: Monday through Friday: 8am - 5pm
- No charge for evaluation
